Spare Forms

You should already have had a trial and received a joining pack containing these forms:

• Membership Form for New Members (includes Codes of Conduct for swimmers and parents).
• Medical Questionnaire
  Required for each new member. It is your responsibility to ensure that the Club is informed of any changes to your child’s medical information.

Subscription

We use LoveAdmin to manage membership and to collect fees via Direct Debit. This is managed by London Zurich. When you’re ready to start the registration process, click on this link to be transferred to LoveAdmin.
